2. Satu pihak yang sama merupakan Pengendali dari beberapa Emiten;
3. Emiten memiliki ketergantungan keuangan dengan Emiten lain;
4. Emiten menerbitkan jaminan untuk mengambil alih dan/atau melunasi sebagian atau seluruh kewajiban Emiten
lain dalam hal Emiten lain tersebut gagal memenuhi kewajiban kepada Kreditur;
5. Direktur, Komisaris dan/atau Pejabat Eksekutif Emiten menjadi Direktur dan/atau Komisaris pada Emiten lain.
Dengan ini Perusahaan Tercatat menyatakan bahwa laporan Perusahaan Tercatat dalam satu grup Perusahaan
yang disampaikan dalam Form ini adalah lengkap dan benar dan Perusahaan Tercatat bertanggung jawab atas
informasi di dalamnya
