MEMUTUSKAN/RESOLVE Guna mememuhi ketentuan Peraturan Otoritas Jasa Keuangan No. 56/POJK.04.2015 Tentang Pembentukan Dan Pedoman Penyusunan Piagam Unit Audit Internal (“POJK No. 56/2015”), dengan ini memutuskan sebagai berikut : e Menunjuk/mengangkat Kepala Unit Audit Internal Perseoan merangkap anggota, yaitu Henny Soetio, yang menggantikan Wati, # Penunjukan/pengangkatan/penggantian Kepala Unit Audit Internal Perseroan tersebut di atas berlaku efektif sejak tanggal Keputusan Direksi ini. Keputusan Direksi ini menggantikan Keputusan Direksi sebelumnya yang mengatur hal yang sama. Memberikan Kuasa kepada salah satu anggota Direksi Perseroan untuk menghadap pada Notaris yang akan menyatakan Keputusan ini dalam akta Notaris (jika diperlukan) In order to comply with Regulation of Otoritas Jasa Keuangan (Financial Services Authority) No. 56/POJK.04.2015 concerning the Forming and Guidance In The Composing Of Internal Audit Unit Charter (“POJK No. 56/2015”), herewith resolves the followings : e To appoint the Head of Internal Audit Unit ofthe Company and also its member, namely Henny Soetio, replacing Wati, e The appointment/replacement of the said Head of Internal Audit Unit shall be effective as of the date of this Board of Director's Decision. This Board of Director's Decision is replacing the previous Board of Director 's Decision governing the same matter. To authorize one of the members of the Board of Directors of the Company to appear before a Notary who will restate this Decision in a Notarial Deed (if reguired).
